ABSTRACT

Objective To discuss the quality control measures of the thermoluminescent dosimetry system and to verify its propriety of when used in environmental radiation monitoring. Methods The quality control measures included the stability test, the dispersibility screening and the uncertainty evaluation of measurement results of the TLD reader and TLD detector, in addition, the environmental radiation accumulated dose monitoring results obtained by the high pressure ionization chamber were compared with those by the thermoluminescent dosimetry system. Results The variation range of TLD reader’s light source coefficient is within 0.070 ~ 0.073 during the preheating and measuring process, which meets the requirements of stability; The χ2 value of the TLD detector is 2.088, which obeys the normal distribution; The thermoluminescent dosimetry system’s nonlinear response, coefficient of variation and energy response meet the measurement requirements; There is a deviation of −6.58% the largest between the cumulative dose obtained by TLD and the high pressure ionization chamber. Conclusion The quality control measures discussed in this paper can be used as a reference for similar thermoluminescent dosimetry system. Our system has passed all the tests and can meet the environmental radiation cumulative dose monitoring requirements.

ABSTRACT

Abstract Introduction: Panoramic radiography and cone beam computed tomography (CBCT) are very important in the diagnosis of oral diseases, however patients are exposed to the risk of ionizing radiation. This paper describes our study aimed at comparing absorbed doses in the salivary glands and thyroid due to panoramic radiography and CBCT and estimating radiation induced cancer risk associated with those methods. Methods Absorbed doses of two CBCT equipment (i-CAT® Next Generation and SCANORA® 3D) and a digital panoramic device (ORTHOPANTOMOGRAPH® OP200D) were measured using thermoluminescent dosimeters loaded in an anthropomorphic phantom on sublingual, submandibular, parotid and thyroid glands. Results Absorbed doses in the i-CAT® device ranged between 0.02 (+/-0.01) and 2.23 mGy (+/-0.03), in the SCANORA™ device ranged from 0.01 (+/-0.01) to 2.96 mGy (+/-0.29) and in the ORTHOPANTOMOGRAPH® OP200D ranged between 0.04 mGy and 0.78 mGy. The radiation induced cancer risk was highlighted in the salivary glands, which received higher doses. The protocols that offer the highest risk of cancer are the high resolution protocols of CBCT equipment. Conclusion CBCT exposes patients to higher levels of radiation than panoramic radiography, so the risks and benefits of each method should be considered. The doses in CBCT were dependent on equipment and exposure parameters, therefore adequate selection minimizes the radiation dose.

ABSTRACT

Abstract Objective: To estimate the entrance surface air kerma (Ka,e) and air kerma in the region of radiosensitive organs in radiographs of pediatric paranasal sinuses. Materials and Methods: Patient data and irradiation parameters were collected in examinations of the paranasal sinuses in children from 0 to 15 years of age at two children's hospitals in the city of Recife, PE, Brazil. We estimated the Ka,e using the X-ray tube outputs and selected parameters. To estimate the air kerma values in the regions of the eyes and thyroid, we used thermoluminescent dosimeters. Results: The Ka,e values ranged from 0.065 to 1.446 mGy in cavum radiographs, from 0.104 to 7.298 mGy in Caldwell views, and from 0.113 to 7.824 mGy in Waters views. Air kerma values in the region of the eyes ranged from 0.001 to 0.968 mGy in cavum radiographs and from 0.011 to 0.422 mGy in Caldwell and Waters views . In the thyroid region, air kerma values ranged from 0.005 to 0.932 mGy in cavum radiographs and from 0.002 to 0.972 mGy in Caldwell and Waters views. Conclusion: The radiation levels used at the institutions under study were higher than those recommended in international protocols. We recommend that interventions be initiated in order to reduce patient exposure to radiation and therefore the risks associated with radiological examination of the paranasal sinuses.


Resumo Objetivo: Estimar o kerma no ar na superfície de entrada (Ka,e) e o kerma no ar na região de órgãos radiossensíveis em radiografias pediátricas dos seios paranasais. Materiais e Métodos: Foram coletados os dados dos pacientes e parâmetros de irradiação em exames de seios paranasais de crianças com 0 a 15 anos, em dois hospitais infantis na cidade do Recife, PE. O Ka,e foi estimado utilizando os rendimentos dos tubos de raios X e os parâmetros selecionados. Os valores de kerma no ar nas regiões dos olhos e tireoide foram estimados utilizando dosímetros termoluminescentes. Resultados: Os valores de Ka,e variaram de 0,065 a 1,446 mGy para radiografias cavum, 0,104 a 7,298 mGy para Caldwell e 0,113 a 7,824 mGy para Waters. Os valores de kerma no ar na região dos olhos variaram de 0,001 a 0,968 mGy para cavum e 0,011 a 0,422 mGy para Caldwell e Waters. Na região tireóidea, os valores de kerma no ar variaram de 0,005 a 0,932 mGy para cavum e 0,002 a 0,972 mGy para Caldwell e Waters. Conclusão: Os parâmetros de irradiação utilizados pelas instituições foram mais elevados do que os recomendados em protocolos britânicos. Ações de otimização são recomendadas de modo a contribuir para a redução da radiação nos pacientes e, portanto, os riscos.

ABSTRACT

PURPOSE: The aim of this study was to compare the effective dose for imaging of mandible between multi-detector computed tomography (MDCT) and cone-beam computed tomography (CBCT). An MDCT with low dose technique was also compared with them. MATERIALS AND METHODS: Thermoluminescent dosimeter (TLD) chips were placed at 25 organ sites of an anthropomorphic phantom. The mandible of the phantom was exposed using 2 different types of MDCT units (Somatom Sensation 10 for standard-dose MDCT, Somatom Emotion 6 for low-dose MDCT) and 3 different CBCT units (AZ3000CT, Implagraphy, and Kavo 3D eXaM). The radiation absorbed dose was measured and the effective dose was calculated according to the ICRP 2007 report. RESULTS: The effective dose was the highest for Somatom Sensation 10 (425.84 microSv), followed by AZ3000CT (332.4 microSv), Somatom Emotion 6 (199.38 microSv), and 3D eXaM (111.6 microSv); it was the lowest for Implagraphy (83.09 microSv). The CBCT showed significant variation in dose level with different device. CONCLUSION: The effective doses of MDCTs were not significantly different from those of CBCTs for imaging of mandible. The effective dose of MDCT could be markedly decreased by using the low-dose technique.

ABSTRACT

RESUMEN El trabajo describe los resultados del servicio de dosimetría personal del Laboratorio de Dosimetría Externa del Centro de Protección e Higiene de las Radiaciones en el período 2006-2009. Se analizan los datos sobre el número de trabajadores ocupacionalmente expuestos que son monitoreados y los niveles de exposición ocupacional que reciben, comparándolos con períodos anteriores y los valores publicados por el UNSCEAR. Los trabajadores monitoreados laboran en la medicina, la industria convencional, la investigación y la docencia. El sector de la salud abarca el 90%. El número de trabajadores monitoreados mantuvo una tendencia creciente en el período, siendo el caso más relevante la práctica de radiodiagnóstico, la cual se duplicó. El número de trabajadores ocupacionalmente expuestos, fue inferior al 50% del total monitoreado, mostrando una tendencia decreciente con respecto a años anteriores. La dosis anual promedio para todas las prácticas fue inferior a 1 mSv, mostrando igualmente una disminución con respecto al período anterior.


ABSTRACT The work describes the results of the personal dosimetry service carried out by the External Dosimetry of the Center for Radiation Protection and Hygiene from 2006 to 2009. Data about number of monitored occupationally exposure workers and occupational radiation exposure levels are analyzed, which are also compared with previously period and UNSCEAR reports. The results show that the number of occupational exposure workers is increase in the period; only about 50% of monitored workers were exposed. The average individual values of annual effective doses were less than 1 mSv for all monitored practices. The comparison with previously period showed a decreasing tendency in the number of exposed workers and in the average annual doses.

OBJETIVO: Determinar, por simulação Monte Carlo, os espectros de feixes de cobaltoterapia em profundidade na água e fatores de correção para doses absorvidas em dosímetros termoluminescentes de fluoreto de lítio. MATERIAIS E MÉTODOS: As simulações dos espectros secundários da fonte clínica de cobalto-60 foram realizadas com o código Monte Carlo PENELOPE, em diversas profundidades na água. Medidas experimentais de dose profunda foram obtidas com dosímetros termoluminescentes e câmara de ionização em condições de referência em radioterapia. Os fatores de correção para os dosímetros termoluminescentes foram obtidos através da razão entre as absorções relativas ao espectro de baixa energia e ao espectro total. RESULTADOS: A análise espectral em profundidade revelou a existência de espectros secundários de baixa energia responsáveis por uma parcela significativa da deposição de dose. Foram observadas discrepâncias de 3,2 por cento nas doses medidas experimentalmente com a câmara de ionização e com os dosímetros termoluminescentes. O uso dos fatores de correção nessas medidas permitiu diminuir a discrepância entre as doses absorvidas para, no máximo, 0,3 por cento. CONCLUSÃO: Os espectros simulados permitem o cálculo de fatores de correção para as leituras de dosímetros termoluminescentes utilizados em medidas de dose profunda, contribuindo para a redução das incertezas associadas ao controle de qualidade de feixes clínicos em radioterapia.


OBJECTIVE: To calculate spectra of cobalt-60 beam at water depth and correction factors for absorbed dose measurements obtained with lithium fluoride thermoluminescent dosimeters using Monte Carlo simulation. MATERIALS AND METHODS: The simulations of secondary spectra of clinical cobalt-60 sources were performed with the PENELOPE Monte Carlo code at different water depths. Experimental measurements of deep doses were obtained with thermoluminescent dosimeters and ionization chamber under reference conditions for radiotherapy. Correction factors for the thermoluminescent dosimeters detectors were obtained through the ratio between the relative energy absorption for the low energy spectrum and the total spectrum. RESULTS: Deep spectral analysis has demonstrated the presence of secondary low-energy spectra responsible for a significant portion of the dose deposition. Discrepancies of 3.2 percent were observed among the doses measured with ionization chamber and thermoluminescent dosimeters. The adoption of correction factors has allowed a reduction in the discrepancy among absorbed doses to a maximum of 0.3 percent. CONCLUSION: Simulated spectra allow the calculation of correction factors for reading of thermoluminescent dosimeters utilized in the measurement of deep doses, contributing for the reduction of uncertainties associated with quality control of clinical beams in radiotherapy.

OBJETIVO: O objetivo deste trabalho foi desenvolver um sistema dosimétrico termoluminescente capaz de avaliar as doses administradas ao reto de pacientes submetidas a braquiterapia de alta taxa de dose para o tratamento do câncer do colo uterino. MATERIAIS E MÉTODOS: O material termoluminescente utilizado para a avaliação da dose no reto foi o LiF:Mg,Ti,Na na forma de pó. O pó foi separado em pequenas porções de 34 mg, que foram acomodadas em um tubo capilar. Este tubo foi colocado em uma sonda retal, que era introduzida no reto da paciente. RESULTADOS: As doses administradas ao reto de seis pacientes submetidas a braquiterapia de alta taxa de dose para o tratamento do câncer do colo uterino foram avaliadas com dosímetros termoluminescentes e apresentaram boa concordância com os valores planejados, com base em duas radiografias ortogonais da paciente, imagens ântero-posterior e lateral. CONCLUSÃO: O sistema de dosimetria termoluminescente utilizado no presente trabalho é simples e de fácil utilização quando comparado a outros métodos de dosimetria do reto. Ele mostrou-se eficiente na avaliação da dose no reto de pacientes submetidas a braquiterapia de alta taxa de dose para o tratamento do câncer do colo uterino.


OBJECTIVE: The present study was aimed at developing a thermoluminescent dosimetric system capable of assessing the doses delivered to the rectum of patients submitted to high-dose-rate brachytherapy for uterine cervix cancer. MATERIALS AND METHODS: LiF:Mg,Ti,Na powder was the thermoluminescent material utilized for evaluating the rectal dose. The powder was divided into small portions (34 mg) which were accommodated in a capillary tube. This tube was placed into a rectal probe that was introduced into the patient's rectum. RESULTS: The doses delivered to the rectum of six patients submitted to high-dose-rate brachytherapy for uterine cervix cancer evaluated by means of thermoluminescent dosimeters presented a good agreement with the planned values based on two orthogonal (anteroposterior and lateral) radiographic images of the patients. CONCLUSION: The thermoluminescent dosimetric system developed in the present study is simple and easy to be utilized as compared to other rectal dosimetry methods. The system has shown to be effective in the evaluation of rectal doses in patients submitted to high-dose-rate brachytherapy for uterine cervix cancer.

ABSTRACT

OBJECTIVES: A large-scale survey of doses to patients undergoing the most frequent radiological examinations was carried out in health services in São Paulo (347 radiological exami nations per 1 000 inhabitants), the most populous Brazilian state. METHODS: A postal dosimetric kit with thermoluminescence dosimeters was used to evalu ate the entrance surface dose (ESD) to patients. A stratified sampling technique applied to the national health database furnished important data on the distribution of equipment and the an nual number of examinations. Chest, head (skull and sinus), and spine (cervical, thoracic, and lumbar) examinations were included in the trial. A total of 83 rooms and 868 patients were in cluded, and 1 415 values of ESD were measured. RESULTS: The data show large coefficients of variation in tube charge, giving rise to large variations in ESD values. Also, a series of high ESD values associated with unnecessary lo calizing fluoroscopy were detected. Diagnostic reference levels were determined, based on the 75th percentile (third quartile) of the ESD distributions. For adult patients, the diagnostic ref erence levels achieved are very similar to those obtained in international surveys. However, the situation is different for pediatric patients: the ESD values found in this survey are twice as large as the international recommendations for chest radiographs of children. CONCLUSIONS: Despite the reduced number of ESD values and rooms for the pediatric pa tient group, it is recommended that practices in chest examinations be revised and that specific national reference doses and image quality be established after a broader survey is carried out.


OBJETIVOS: Estudio a gran escala de las dosis aplicadas a pacientes sometidos a los exámenes radiológicos más frecuentes realizados en los servicios de salud de São Paulo (347 exámenes radiológicos por 1 000 habitantes), el estado más poblado de Brasil. MÉTODOS: Se empleó un sistema dosimétrico postal con dosímetros termoluminiscentes para evaluar la dosis en la superficie de entrada (DSE) de los pacientes. Se realizó un muestreo estratificado de la base de datos nacional de salud que aportó información importante sobre la distribución del equipamiento y el número de pruebas realizadas anualmente. El estudio abarcó los exámenes de tórax, cabeza (cráneo y senos paranasales) y columna vertebral (cervical, dorsal y lumbar). Se incluyeron 83 instalaciones y 868 pacientes; se obtuvieron 1 415 mediciones de DSE. RESULTADOS: Los resultados mostraron grandes coeficientes de variación en la carga del tubo, lo que originó variaciones considerables en las DSE. Además, se detectaron valores elevados de DSE asociados con fluoroscopias de localización innecesarias. Los niveles de referencia diagnósticos se determinaron a partir del percentil 75 (tres cuartiles) de las distribuciones de las DSE. Para pacientes adultos, los niveles de referencia diagnósticos alcanzados fueron muy similares a los obtenidos en estudios internacionales. No obstante, la situación fue diferente en los pacientes pediátricos: los valores de DSE obtenidos en este estudio fueron dos veces mayores que los recomendados internacionalmente para radiografías de tórax en niños. CONCLUSIONES: A pesar del reducido número de valores de DSE e instalaciones en el grupo de pacientes pediátricos, se recomienda revisar la práctica de estudios radiológicos de tórax y establecer referencias nacionales específicas para las dosis y la calidad de las imágenes, después de realizar un estudio más amplio.

OBJETIVO: Este trabalho avalia a distribuição de dose recebida por médicos envolvidos em procedimentos hemodinâmicos de angioplastia coronária e coronariografia. A influência de alguns fatores, como o modo de fluoroscopia pulsado ou contínuo e o local de acesso à veia e/ou artéria, foi investigada. MATERIAIS E MÉTODOS: Para esta avaliação foram feitas medições utilizando dosímetros termoluminescentes de LiF:Mg,Ti, posicionados em sete diferentes pontos do corpo dos profissionais: mãos, joelho, pescoço, testa e tórax, por dentro e por fora do avental de chumbo. A dose foi avaliada, por exame, nos médicos que executaram os procedimentos (30 de angioplastia e 60 de coronariografia). Os dosímetros termoluminescentes foram calibrados na grandeza operacional equivalente de dose pessoal, Hp(d), nas profundidades de 0,07, 3 e 10 mm. RESULTADOS: Os resultados mostram a importância do uso do protetor de tireóide e avental de chumbo para a redução da dose recebida pelos médicos. As doses dos profissionais que executaram procedimentos por via braquial usando modo contínuo de fluoroscopia foram mais altas do que os que executaram por via femoral e modo pulsado de fluoroscopia. CONCLUSÃO: Este estudo mostra a necessidade de medidas adicionais de proteção e a implementação de mecanismos de treinamento em proteção radiológica para os médicos que trabalham com cardiologia intervencionista.


OBJECTIVE: The present study evaluates the dose distribution received by physicians involved in hemodynamic procedures of coronary angiography and coronariography. The influence of some factors such as pulsed or continuous fluoroscopy mode and vein and/or artery access site was investigated. MATERIALS AND METHODS: Measurements have been performed with LiF:Mg,Ti thermoluminescent dosimeters placed on seven different sites of the practitioners' bodies: hands, left knee, neck, forehead and chest, under and over the lead apron. Radiation doses for each procedure were evaluated in physicians who had performed 60 coronariography and 30 angioplasty procedures. The thermoluminescent dosimeters were calibrated in the operational quantity personal dose equivalent, Hp(d), at depths of 0.07, 3 and 10 mm. RESULTS: The results of the present study demonstrate the significant role played by thyroid protectors and lead aprons for reducing radiation doses received by practitioners. Occupational doses in continuous mode fluoroscopy through brachial access were higher than with pulsed mode fluoroscopy through femoral access. CONCLUSION: The present study has demonstrated the necessity of implementing additional protective measures as well as a mechanism of training in radiation protection for physicians involved in interventional cardiological procedures.

ABSTRACT

PURPOSE: The purpose of this study was to measure the absorbed dose and to calculate the effective dose for periapical radiography done by portable intraoral x-ray machines. MATERIALS AND METHODS: 14 full mouth, upper posterior and lower posterior periapical radiographs were taken by wall-type 1 and portable type 3 intraoral x-ray machines. Thermoluminescent dosemeters were placed at 23 sites at the layers of the tissue-equivalent ART woman phantom for dosimetry. Average tissue absorbed dose and radiation weighted dose were calculated for each major anatomical site. Effective dose was calculated using 2005 ICRP tissue weighted factors. RESULTS: On 14 full mouth periapical radiographs, the effective dose for wall-type x-ray machine was 30 Sv; for portable x-ray machines were 30 Sv, 22 Sv, 36 Sv. On upper posterior radiograph, the effective dose for wall-type x-ray machine was 4 Sv; for portable x-ray machines doses were 4 Sv, 3 Sv, 5 Sv. On lower posterior radiograph, the effective dose for wall type x-ray machine was 5 Sv; for portable x-ray machines doses were 4 Sv, 4 Sv, 5 Sv. CONCLUSION: Effective doses for periapical radiographs performed by portable intraoral x-ray machines were similar to doses for periapical radiographs taken by wall type intraoral x-ray machines.
